Sponsor Registration

This form is for Catholics are are least 16 years of age who have been asked to be one of the following:

the Sponsor of a child in 3rd grade who will be confirmed and receive their First Eucharist
the Sponsor of a teen (RCI-C) or adult (RCI-A) in the process to become Catholic
the Sponsor of an adult Catholic who still needs to receive Confirmation
